Kotraba Insurance Agency LLC complies with the Fair Credit Reporting Act ("FCRA"), 15 U.S.C. § 1681 et seq., as amended. This notice explains how your credit and consumer report information may be used in connection with your insurance inquiry.
Information obtained from a consumer reporting agency, including your consumer credit report, is relevant to your insurance transaction and may be used in underwriting—including determining your eligibility for coverage, premium rates, and policy terms. This use complies with applicable federal and state law.
Under the FCRA, you have the right to request a free copy of your consumer credit report once every 12 months. You may access your free annual credit report at AnnualCreditReport.com, the only official website authorized by the Federal Trade Commission.
If you believe any information in your consumer report is inaccurate or incomplete, you have the right to dispute that information directly with the consumer reporting agency that provided the report. Each of the three major nationwide consumer reporting agencies has a dedicated process for handling disputes:
If action is taken against you, or if a premium rate differential is offered that is unfavorable to you, based in whole or in part on information contained in your consumer report, we will provide you with notice of that adverse action and the name and contact information of the consumer reporting agency that furnished the report, as required by federal law.
If you have questions about how we handle your information or your rights under the FCRA, please contact us:
